How to request my records/sign up for Patient Fusion
Call your provider’s office and request online access to your health records or find a Practice Fusion provider. To view your medical records in Patient Fusion, your provider must use Practice Fusion as their electronic health record (EHR) system and must grant you access to your records.
If your provider has already granted you access, follow these steps to set up your account.

To view your medical records in Patient Fusion, your provider must use Practice Fusion as their electronic health record (EHR) system and must grant you access to your records. Once the provider has granted you access to Patient Fusion, the provider will provide you with your unique PIN.
When your provider’s office grants you access, a registration email will be sent to the email address on file with your provider’s office. Within the message, click Access your records (If you're having trouble locating the email, see How can I locate a missing registration email?)

Accessing your health records
After logging in and reviewing your record updates and pending appointments on your Dashboard, click on Health Records to see your up-to-date health information. Please note that this will only include what your doctor has added into the system. If you see incomplete or incorrect records, contact your doctor’s office.

You can download your records to give to another doctor by clicking on the Download button in the top right. Choose ‘PDF format’, select the information you want downloaded, then click Download record. This can be printed and given to a different doctor at any time.
Reviewing your appointments and scheduling a new one
The Appointments tab includes all of your upcoming appointments on your doctor’s schedule, as well as any past appointments you’ve had with them. You can view visit summaries of past appointments by clicking on the appointment time under the Past tab.
Requesting a new appointment from your account is easy. Simply click on Providers on the top of the screen, then find your provider in the list of doctors in the medical practice. Click on Book an appointment to go to your doctor’s online booking profile, where you can request an appointment time if your doctor has enabled online booking.

You will receive an email when your doctor has confirmed or denied your request. You can also find the phone number of your doctor’s office on the Providers tab to follow up on your request.
If your doctor's office has enabled patient messaging on their side, you will have a Messages section available in your Patient Fusion account.
Note: If the Messages section is not present, this means the doctor's office will need to first enable patient messaging on their end. Please reach out to your doctor's office directly to request this.
Please be aware: Messages are only for non-urgent issues. Call your doctor's office if you need an immediate response. Dial 911 for life-threatening situations.
